To get your Michigan Medical Marijuana License, you must then obtain a
Physician Certification Form
Schedule an in-person examination with a Healthy Partners Medical Marijuana Doctor. If you have already received a diagnosis of qualifying condition, then you should fax or bring your records with you to your appointment. If you do not have access to your medical records, or if you have not yet received a diagnosis for your condition, then a Healthy Partners MMMP Doctor will determine your eligibility during an in-person physical examination. Diagnosing certain conditions may require additional testing.
Once you have your Physician Certification Form, you will mail it, other required documents, and your check or money order for the State application fees. The State of Michigan is required to process all applications within 15 days of receipt. Once an application is approved, most patients receive their Medical Marijuana Card within 2-3 weeks.
