Michigan Medical Marijuana FAQ

The Details
  • First, in order for you to get your card the same day, you must submit the application online (we will provide you with the instructions).
  • Second, there is a daily time cutoff for this service.  Applications submitted online before 6:00 PM will be confirmed by us the same day. Otherwise, we will confirm your application the following morning.
  • Third, even those who have a caregiver can take advantage of our Same Day Card Offer. You merely have to complete and mail an “Add or Change Caregiver Amendment” form once you receive the “Approval” email from the state.
Beware of “Full Service” offers by other clinics Some patients have asked if we can submit the application online for them. The State of Michigan Cannabis Regulatory Agency (CRA) is clear: Patients should not allow other individuals or third parties to submit their applications or any other documents to the MMMP as patients will be unable to determine when or if their applications were submitted.

In a word, no. While Healthy Partners medical marijuana doctors do accept various insurance plans, most insurance providers do not cover Medical Marijuana Doctor appointments. We do accept cash, credit cards, and Google/Apple Pay

Healthy Partners accepts Cash, Credit/Debit cards, and Google/Apple Pay.  We DO NOT accept checks.

Debilitating medical (“Qualifying”) condition means any of the following conditions and/or their treatments:

·         Agitation of Alzheimer’s disease
·         Acquired Immune Deficiency Syndrome (AIDS)
·         Amyotrophic Lateral Sclerosis (ALS – Lou Gehrig’s Disease)
·         Arthritis
·         Autism
·         Cancer
·         Cerebral Palsy
·         Crohn’s disease
·         Chronic Pain
·         Colitis
·         Glaucoma
·         Positive status for Human Immunodeficiency Virus (HIV)
·         Hepatitis C
·         Inflammatory Bowel Disease
·         Nail patella
·         Obsessive Compulsive Disorder
·         Parkinson’s
·         Post Traumatic Stress Disorder (PTSD)
·         Rheumatoid Arthritis
·         Spinal Cord Injury
·         Tourette’s Syndrome
·         Ulcerative Colitis

A patient also qualifies if s/he experiences any chronic or debilitating disease or medical condition, or it’s treatment, which produces one or more of the following symptoms or side effects:

·         Cachexia or wasting syndrome
·         Severe and chronic pain
·         Nausea
·         Seizures, including but not limited to those characteristic of epilepsy
·         Severe and persistent muscle spasms, including but not limited to those characteristic of multiple sclerosis

Virtual appointments are conducted via Zoom. You should have reliable internet or mobile data service and a connected camera for virtual appointments.

Telemedicine appointments are conducted over the phone.

Renewal AND Annual Follow Up Appointments: Please bring the following:

  • Your proof of Michigan residency. The State of Michigan will ONLY accept the following as residency proof:
    • A valid Michigan driver’s license
    • Michigan personal identification card
    • A State or Federally issued picture ID (including passport) WITH your Michigan voter registration card.
  • You must also bring your Michigan Medical Marijuana Patient Card.
  • We accept Cash, Credit/Debit and Google/Apple Pay.

 

With Medical Records Appointments: 

Please bring the following:

  • Your proof of Michigan residency. The State of Michigan will ONLY accept the following as residency proof:
    • A valid Michigan driver’s license
    • Michigan personal identification card
    • A State or Federally issued picture ID (including passport) WITH your Michigan voter registration card.
  • Your Medical Records – most recent visit summary will suffice
  • Form of payment: Cash, Credit/Debit or Google/Apple Pay.

 

No Medical Records Appointments: 

Please bring the following:

  • Your proof of Michigan residency. The State of Michigan will ONLY accept the following as residency proof:
    • A valid Michigan driver’s license
    • Michigan personal identification card
    • A State or Federally issued picture ID (including passport) WITH your Michigan voter registration card.
  • Form of payment: Cash, Credit/Debit or Google/Apple Pay.

 

 

Denial Letter Fix: Please bring the following:

  • Your proof of Michigan residency. The State of Michigan will ONLY accept the following as residency proof:
    • A valid Michigan driver’s license
    • Michigan personal identification card
    • A State or Federally issued picture ID (including passport) WITH your Michigan voter registration card.
  • Your Denial Letter – so we have a copy on file as well.

Form of payment: Cash, Credit/Debit or Google/Apple Pay.

If you are running late but will arrive within 15 minutes of your scheduled appointment time, then yes.  If you think that you will be later than 15 minutes, please notify our office so that we can let you know whether we have availability to see you.

Schedule a “Denial Letter Fix” appointment with our office.  This is a free appointment, during which we will get you everything you need to resubmit your application.

We typically ask for the most recent “Visit Summary” from your primary care or specialist physician. Your records should show:
  1. List of ongoing health issues
  2. List of current medications
  3. The diagnosis of your qualifying condition
  4. Your name and date of birth
  5. The diagnosing physician’s name
While you can have the records faxed to us, doctor’s offices can take several weeks to comply with your request. It is best if you can print them and bring them with you to your appointment. NOTE: Showing the doctor your records through the patient portal on your phone is no longer acceptable. We must have the records on file with our office. We cannot accept email attachments for security purposes.

To add or change a Caregiver, simply fill out the “Add or Change Caregiver Amendment” form.  All Michigan Medical Marijuana Program forms can be found here.
To add or change your name or address on your Medical Marijuana Program Patient ID card, simply fill out the “Name or Address Amendment” form.  All Michigan Medical Marijuana Program forms can be found here. If you have an Accela account, you can make these changes online in your Accela dashboard.
There is no standalone caregiver card or license. The only way to become a caregiver is to have a medical marijuana patient designate you as their caregiver. A patient can designate your as their caregiver through either their Application Form for Registry Identification Card or a Add or Change Caregiver Amendment form.  The Application Form for Registry Identification Card should only be used when the patient is applying to the Michigan Medical Marijuana Program as either a new or renewing medical marijuana patient.  The Add or Change Caregiver Amendment form should be used after the patient has received their Marijuana Registry Patient ID Card.
You can request a new Marijuana Registry Patient ID or Caregiver Card from the Marijuana Regulatory Agency at no cost. You should submit the Request Replacement Card(s) Form.  If you have an Accela account, you can request a new card through your Accela dashboard.
